C2C:Equitable Pathways Exit Phase for Members

Goal: To have AmeriCorps members and their Site Supervisors complete all the exit paperwork and send the exit paperwork Campus Compact of Oregon on the member's last day of service.

Steps to accomplishing the goal:
  1. For the last day of your service, schedule an exit interview with your site supervisor
  2. Have your site supervisor complete a C2C Member Exit Survey before the exit interview
  3. Submit your final timesheet in On-Corps Reporting at the end of your last day
  4. Complete your Exit Form.
  5. At the scheduled exit interview:
    • Your Site Supervisor will review the End of Term C2C member evaluation with you, after which you should sign the evaluation
    • Give your site supervisor the following completed exit paperwork (NOTE: Your site supervisor will mail Oregon Campus Compact the exit form and end-of-term CAC member evaluation):
      • Signed C2C Member Exit Survey
      • Signed C2C Supervisor Exit Survey
      • Completed exit form.
      • Sustainability Binder/Plan
      • Email supervisor draft copy of the progress report (this should have been done before the C2C Final Progress Report was submitted)
