CAC Exit Phase
Goal: To have AmeriCorps members complete all the exit paperwork and to have the site supervisor mail the exit paperwork to WA/OR Campus Compact on the member's last day of service.

- For the last day of your service, schedule an exit interview with your site supervisor
- Have your site supervisor complete an End of Term CAC Member evaluation before the exit interview
- Submit your final timesheet in On-Corps Reporting at the end of your last day
- Complete your Exit Form.
- At the scheduled exit interview:
- Your Site Supervisor will review the End of Term CAC member evaluation with you, after which you should sign the evaluation
- Give your site supervisor the following completed exit paperwork (NOTE: Your site supervisor will mail Oregon Campus Compact the exit form and end-of-term CAC member evaluation):
- Signed End of Term CAC member evaluation
- Completed exit form.
- Sustainability Binder/Plan
- "6 folders" Please make sure they are properly labeled and have the correct grant year, 2018-19.
- Email supervisor draft copy of the progress report (this should have been done before the CAC Final Progress Report was submitted)
