C2C:Equitable Pathways Exit Phase for Site Supervisors
Goal: To have AmeriCorps members and their Site Supervisors complete all the exit paperwork and send the exit paperwork Campus Compact of Oregon on the member's last day of service.

- For the last day of your service, schedule an exit interview with your Member
- Complete a C2C Supervisor Exit Survey before the exit interview
- Have your Member complete a C2C Member Exit Survey before the exit interview
- Approve your Member's final timesheet in On-Corps Reporting at the end of your last day
- Have your Member complete the Exit Form and send it to your Program Director along with the Evaluations
- At the scheduled exit interview:
- You and your Site Supervisor will review the End of Term C2C Member and Site Supervisor evaluations with you, after which you should sign the evaluation
- Collect from your Member the following completed exit paperwork (NOTE: You will send Oregon Campus Compact the exit form and end-of-term CAC Member and Site Supervisor evaluations):
- Signed C2C Member Exit Survey
- Signed C2C Supervisor Exit Survey
- Completed exit form.
- Sustainability Binder/Plan
- Review the draft copy of the progress report (this should have been completed by your Member before the C2C Final Progress Report was submitted), then submit the final version to your Program Director
